# LongevityGLP1 > Monthly prices at the starting dose for compounded tirzepatide, semaglutide, metformin and rapamycin from US telehealth programs, each with a Healthspan Score. Updated October 2026. ## How the Healthspan Score works Each provider gets a Healthspan Score, 0 to 100, per drug on its menu, and four checks make up the total: Price (35 points), Care (25 points), Pharmacy (25 points), Clarity (15 points). We read every check off the provider's public pricing, plan and pharmacy pages, never an ad or a promo lander. Lineups follow the score. If two providers land on the same total with the same Price points, the Partner is listed ahead; the Partner label itself never adds or removes a point. 26 of the 120 providers on file are Partners. FDA declared the semaglutide shortage over on February 2025. Pharmacies lost their broad permission to make copies in spring 2025. https://longevityglp1.com/drugs/compounded-semaglutide - Compounded tirzepatide — No FDA approval; licensed compounding pharmacies may make it (status as of October 2026). Pharmacies lost their shortage-era allowance in March 2025: 503A after a March 5 court ruling, 503B on March 19. https://longevityglp1.com/drugs/compounded-tirzepatide - Liraglutide — FDA-approved (status as of October 2026). FDA-approved generics now exist for both Victoza and Saxenda, and FDA still lists liraglutide injection in shortage. https://longevityglp1.com/drugs/liraglutide - Metformin — FDA-approved for another condition (status as of October 2026). Metformin is FDA-approved only to improve blood sugar control in type 2 diabetes; any use for weight, prediabetes or aging is off-label. https://longevityglp1.com/drugs/metformin - Mounjaro — FDA-approved (status as of October 2026). FDA approved Mounjaro on May 2022 to improve blood sugar control in adults with type 2 diabetes; in the US it is not approved for weight loss. https://longevityglp1.com/drugs/mounjaro - Oral semaglutide — FDA-approved (status as of October 2026). FDA approved Wegovy tablets on December 2025 to reduce weight and keep it off, and to lower heart-event risk in adults with heart disease and excess weight. https://longevityglp1.com/drugs/oral-semaglutide - Orforglipron — FDA-approved (status as of October 2026). FDA approved Foundayo on April 2026 for weight management in adults. https://longevityglp1.com/drugs/orforglipron - Ozempic — FDA-approved (status as of October 2026). FDA approved rapamycin (sirolimus) to prevent kidney-transplant rejection in people 13 and older, and later for a rare lung disease, LAM. It carries a boxed warning for infection and lymphoma. https://longevityglp1.com/drugs/rapamycin - Retatrutide — No FDA approval (status as of October 2026). Retatrutide is not FDA-approved for any use, and FDA says it cannot be used in compounding under federal law. https://longevityglp1.com/drugs/retatrutide - Semaglutide — FDA-approved (status as of October 2026). Compounded semaglutide isn't FDA-approved, and the shortage that allowed mass copies ended February 2025. https://longevityglp1.com/drugs/semaglutide - Tirzepatide — FDA-approved (status as of October 2026). No tirzepatide tablet is approved, and compounded tirzepatide isn't FDA-approved and is legal only in narrow cases since 2025. https://longevityglp1.com/drugs/tirzepatide - Wegovy — FDA-approved (status as of October 2026). Its 2026 review pooled 91 placebo-controlled trials with 107,910 people and found no rise in suicidal thoughts or behavior, or in anxiety, depression, irritability or psychosis, on GLP-1 drugs. FDA then asked for the warning to come off the Wegovy, Saxenda and Zepbound labels. If your mood changes on any medicine, tell your prescriber. Updated October 2026. - [Alcohol on a GLP-1: what the labels say, and what they don't](https://longevityglp1.com/research/can-you-drink-alcohol-on-glp1): The labels for Ozempic, Wegovy, Zepbound and Mounjaro don't set an alcohol rule. Their warnings still matter if you drink: pancreatitis, dehydration from vomiting or diarrhea, and low blood sugar when combined with insulin or a sulfonylurea. Metformin's label does warn against heavy drinking, because alcohol raises its lactic-acidosis risk. Ask your prescriber what's safe for you. Updated October 2026. - [GLP-1 drugs and pregnancy: what the Ozempic, Wegovy and Zepbound labels say](https://longevityglp1.com/research/can-you-take-glp1-while-pregnant): No. The Wegovy and Zepbound labels say they may cause fetal harm and should be stopped once pregnancy is recognized. Ozempic and Wegovy labels say to stop at least 2 months before a planned pregnancy, because semaglutide stays in the body. Zepbound and Mounjaro can make birth-control pills less reliable for 4 weeks after starting and after each dose increase. Updated October 2026. - [GLP-1 drugs and muscle: how much of the weight lost is lean mass](https://longevityglp1.com/research/do-glp1s-cause-muscle-loss): Some, as with any weight loss. In a SURMOUNT-1 body-scan study, people on tirzepatide lost 33.9% of their fat and 10.9% of their lean mass, so about three-quarters of the weight lost was fat and a quarter lean, the same split seen on placebo. The Zepbound and Wegovy labels say fat loss is greater than lean loss. Updated October 2026. - [GLP-1 drugs and blood pressure: the drop in trials, and the low-pressure risk](https://longevityglp1.com/research/do-glp1s-lower-blood-pressure): In trials, yes, a little. Zepbound lowered systolic blood pressure by about 7 mmHg over 72 weeks and Wegovy by about 6 mmHg over 68 weeks, compared with about 1 mmHg on placebo. Neither drug is approved to treat high blood pressure. Both labels also report low blood pressure in 1% to 2% of people, often with dehydration. Updated October 2026. - [Medicare and Zepbound or Wegovy: the $50 GLP-1 Bridge, explained](https://longevityglp1.com/research/does-medicare-cover-zepbound): Yes, if you qualify. Since July 2026, Medicare's GLP-1 Bridge covers Wegovy (shot or pill), Zepbound KwikPen and Foundayo for weight management at a $50 copay through December 2027. You need a qualifying BMI and condition, plus prior authorization. With type 2 diabetes, sleep apnea or fatty liver disease, your Part D plan decides instead. Updated October 2026. - [Ozempic storage: the fridge before first use, then 56 days](https://longevityglp1.com/research/does-ozempic-need-to-be-refrigerated): Before first use, yes. Keep new Ozempic pens in the fridge at 36°F to 46°F, away from the cooling element, and never frozen. After the first injection, a pen keeps for 56 days at 59°F to 86°F or in the fridge, cap on, out of heat and sunlight. Then it's thrown away. Updated October 2026. - [Zepbound and tiredness: how common it is, and what drives it](https://longevityglp1.com/research/does-zepbound-make-you-tired): It can. In Zepbound's weight-loss trials, 5% to 7% of people reported fatigue (the label counts weakness, lethargy and malaise together), compared with 3% on placebo. It was a little more common at higher doses. Tiredness can also come with nausea, vomiting or diarrhea and the dehydration they cause, which are most common while the dose is rising. Updated October 2026. - [Zepbound storage: the fridge, the 21-day rule and the KwikPen's 30 days](https://longevityglp1.com/research/does-zepbound-need-to-be-refrigerated): Yes. Keep Zepbound in the fridge at 36°F to 46°F in its carton. If needed, a single-dose pen or vial can stay unrefrigerated at up to 86°F for a total of 21 days, then it's thrown away. KwikPens and multi-dose vials follow a 30-day limit at room temperature or after first use, or four weekly doses, whichever comes first. Updated October 2026. - [Hair loss on Ozempic, Wegovy, Zepbound and Mounjaro: what the labels report](https://longevityglp1.com/research/glp1-hair-loss): Yes, it can. In trials, hair loss affected 4% to 5% of people on Zepbound and 3.3% on Wegovy 2.4 mg, against 1% on placebo, and both labels tie it to weight reduction. Women reported it far more often than men. Ozempic and Mounjaro list hair loss only from reports after approval, without a trial rate. Updated October 2026. - [How long the Wegovy pill takes to work, from the 1.5 mg start to week 64](https://longevityglp1.com/research/how-long-does-the-wegovy-pill-take-to-work): Gradually, over months. In the Wegovy pill's main trial, average weight loss reached 13.6% by week 64 on the 25 mg daily dose, against 2.4% on placebo, and about 6 in 10 people lost at least 10%. The pill starts at 1.5 mg a day and is raised over time, so the first weeks show the least change. Updated October 2026. - [How long Wegovy takes to work: what the first months and week 68 looked like](https://longevityglp1.com/research/how-long-does-wegovy-take-to-work): Gradually. In Wegovy's main trial, average weight loss reached 14.9% at week 68 on 2.4 mg, against 2.4% on placebo, and two in three people lost at least 10%. The first 16 weeks are spent stepping up from the 0.25 mg starting dose, so most of the loss comes after that. People with type 2 diabetes lost less, about 9.6%. Updated October 2026. - [How long Zepbound stays in your body after the last dose](https://longevityglp1.com/research/how-long-does-zepbound-stay-in-your-system): Tirzepatide, the drug in Zepbound, has a half-life of about 5 days, so half a dose is gone in about 5 days. By the usual five-half-lives rule of thumb, it's mostly cleared 25 to 30 days after the last shot. That's our arithmetic; the label gives only the half-life. Semaglutide lingers longer, about 5 to 7 weeks. Updated October 2026. - [How long Zepbound takes to work, from the first month to week 72](https://longevityglp1.com/research/how-long-does-zepbound-take-to-work): Zepbound works gradually. In its 72-week weight trial, average loss reached 15.0% at 5 mg, 19.5% at 10 mg and 20.9% at 15 mg, against 3.1% on placebo. Weight starts falling within weeks, then keeps falling for months as the dose rises. Your pace depends on dose, tolerance and how long you stay on it. Updated October 2026. - [How much weight people lost on Ozempic in its diabetes trials](https://longevityglp1.com/research/how-much-weight-can-you-lose-on-ozempic): In Ozempic's diabetes trials, people lost about 7 to 14 lb on average, roughly 3% to 6.5% of their body weight, over 30 to 56 weeks. Ozempic isn't approved for weight loss. Wegovy, the same semaglutide labeled for weight, averaged 14.9% at 68 weeks in adults without diabetes and 9.6% in adults with diabetes. Updated October 2026. - [How to get Ozempic: the prescription, the three ways to pay, and what to avoid](https://longevityglp1.com/research/how-to-get-ozempic): You need a prescription. Ozempic is FDA-approved for adults with type 2 diabetes, so a prescriber decides if it fits you. With commercial coverage, the maker's card can bring it to $25. Without it, NovoCare charges $199 for the first two starter fills, then $349 a month, or $499 for 2 mg. It's never over the counter. Updated October 2026. - [Is retatrutide safe? What the trials found, and what FDA has said](https://longevityglp1.com/research/is-retatrutide-safe): It isn't proven yet. Retatrutide is not FDA-approved, so it has no label. In Lilly's trials, the most common side effects were stomach-related, rose with dose and were mostly mild to moderate, and heart rate went up. FDA says retatrutide can't legally be compounded, and products sold as "research use only" are not the drug tested in trials. Updated October 2026. - [Metformin side effects: how common diarrhea is, and what's rare but serious](https://longevityglp1.com/research/metformin-side-effects): Yes, diarrhea is the most common one. In the label's trial, 53% of people on immediate-release metformin had diarrhea versus 12% on placebo; in the extended-release trials it was 10% versus 3%. Nausea, gas and indigestion are next. The label says stomach effects usually ease over time. Lactic acidosis is rare but serious, and long-term use can lower vitamin B12. Updated October 2026. - [Metformin and weight loss: about 2%, and why that's the honest number](https://longevityglp1.com/research/metformin-weight-loss): A little. In the Diabetes Prevention Program, people taking metformin lost about 2% of their body weight versus almost none on placebo, and the loss lasted at least 10 years in those who kept taking it. Metformin isn't FDA-approved for weight loss, and no trial shows a fast result. GLP-1 drugs lost 15% to 21% in their trials. Updated October 2026. - [How much is Mounjaro? Every price, with and without insurance](https://longevityglp1.com/research/mounjaro-cost): Without insurance, Mounjaro costs $499 a month at every dose through LillyDirect. With commercial insurance that covers it, Lilly's savings card can bring it to $25 for a 1-, 2- or 3-month fill. If your commercial plan doesn't cover it, the card price is as low as $499. Medicare Part D plans often cover it for type 2 diabetes. Updated October 2026. - [Mounjaro doses, from the 2.5 mg start to the 15 mg maximum](https://longevityglp1.com/research/mounjaro-dosage): The highest Mounjaro dose is 15 mg once a week for adults (10 mg for children 10 and older). Everyone starts at 2.5 mg weekly, and a prescriber raises it only if blood sugar needs it. It comes in 2.5, 5, 7.5, 10, 12.5 and 15 mg. It's taken once a week, any time of day, with or without meals. Updated October 2026. - [How Ozempic is taken: the weekly dose, from 0.25 mg to 2 mg](https://longevityglp1.com/research/ozempic-dosage): Ozempic is injected once a week, on the same day, at any time, with or without meals. The label starts at 0.25 mg, which isn't a maintenance dose, then keeps people on 0.5, 1 or 2 mg based on blood sugar. The most it allows is 2 mg a week. Your prescriber and the pen instructions cover the rest. Updated October 2026. - [Wegovy doses, from the 0.25 mg start to the 7.2 mg maximum](https://longevityglp1.com/research/wegovy-dosage): Wegovy's label starts at 0.25 mg once a week. Most adults stay on 1.7 mg or 2.4 mg, and 2.4 mg is the recommended weight dose. The highest dose is 7.2 mg weekly, for people who tolerate 2.4 mg and need more weight loss. The Wegovy pill starts at 1.5 mg a day and is kept at 25 mg. Updated October 2026. - [What happens when you stop Ozempic, Wegovy or Zepbound](https://longevityglp1.com/research/what-happens-when-you-stop-taking-glp1): Most people regain weight. One year after stopping semaglutide in the STEP 1 extension, people had regained about two-thirds of the weight they'd lost, and blood pressure and other gains drifted back. In SURMOUNT-4, people switched from tirzepatide to placebo regained 14% of body weight, while those who stayed on lost another 5.5%. Updated October 2026. - [Can you get retatrutide legally? Mainly through a clinical trial](https://longevityglp1.com/research/can-you-get-retatrutide-legally): For most people in the US, the main legal way to get retatrutide right now is to join a clinical trial. It isn't FDA-approved, FDA says it can't be compounded, and the "research use only" vials sold online are illegal unapproved drugs. Lilly also runs a narrow expanded-access program a treating doctor can request for adults with severe obesity. Updated October 2026. - [Compounded semaglutide prices: what you'll pay each month](https://longevityglp1.com/research/compounded-semaglutide-cost): Across online programs, compounded semaglutide runs $75–$349 a month as of this update at each program's ongoing price. Most programs bill less than the $349 Wegovy pen, but the Wegovy pill now starts at $149 through NovoCare. First-month offers and prepaid plans make many advertised prices look lower than what you'll pay. Updated October 2026. - [Compounded semaglutide in the fridge: how long it keeps](https://longevityglp1.com/research/compounded-semaglutide-fridge): An unopened vial of compounded semaglutide lasts in the fridge until the beyond-use date on your pharmacy's label. After the first dose, FDA recommends throwing a multi-dose vial away 28 days later, "even if instructions from the compounder say the medication can be used for longer." Brand Ozempic pens follow different label rules: 56 days after first use. Updated October 2026. - [What’s the highest dose of compounded semaglutide? There’s no label, only brand limits](https://longevityglp1.com/research/compounded-semaglutide-max-dose): Compounded semaglutide has no FDA-approved label, so there’s no official maximum. The approved injections set the reference points: Ozempic’s label maximum is 2 mg once weekly, and Wegovy’s usual maintenance dose is 2.4 mg weekly, with up to 7.2 mg allowed for some patients already on 2.4 mg. Updated October 2026. - [Compounded tirzepatide cost: what programs charge, month by month](https://longevityglp1.com/research/compounded-tirzepatide-cost): Compounded tirzepatide costs $85–$433 a month across online programs as of this update, using each program's ongoing price rather than its first-month offer. Some programs charge more as your dose rises, and prepaid plans lower the monthly figure. Brand Zepbound costs $299 to $449 a month self-pay through LillyDirect, depending on dose and refill timing. Updated October 2026. - [How long compounded tirzepatide can stay out of the fridge](https://longevityglp1.com/research/compounded-tirzepatide-out-of-fridge): Compounded tirzepatide has no FDA label, so the time it can spend unrefrigerated is set by your compounding pharmacy's label. For comparison, Zepbound's label allows single-dose pens and vials up to 21 days at no more than 86°F (30°C). FDA also advises discarding any multi-dose vial 28 days after first use, even if the compounder says longer. Updated October 2026. - [The starting dose of compounded tirzepatide, and why it isn't on a label](https://longevityglp1.com/research/compounded-tirzepatide-starting-dose): Compounded tirzepatide has no FDA label, so there is no official starting dose. Prescribers generally follow Zepbound's label, which starts at 2.5 mg injected once weekly for 4 weeks and says that dose "is not approved as a maintenance dosage." How much liquid that is depends on your pharmacy's vial, so confirm it with your prescriber and pharmacist. Updated October 2026. - [Does compounded semaglutide work as well as Wegovy?](https://longevityglp1.com/research/does-compounded-semaglutide-work): Semaglutide itself works: in Wegovy's STEP 1 trial, adults lost 14.9% of body weight on average over 68 weeks, vs 2.4% on placebo. No trial has tested compounded semaglutide, though. It should work similarly only if the vial holds real semaglutide at its labeled strength, which FDA does not check for compounded drugs. Updated October 2026. - [Does compounded tirzepatide work? The evidence is all from Zepbound](https://longevityglp1.com/research/does-compounded-tirzepatide-work): Tirzepatide itself works well: adults in the SURMOUNT-1 trial lost 15% to 21% of their body weight over 72 weeks. That trial used Lilly’s approved drug. No controlled trial has tested a compounded tirzepatide product, so a compounded version works only as well as it matches that drug. Updated October 2026. - [Medicare and Medicaid coverage for Ozempic in 2026](https://longevityglp1.com/research/does-medicare-cover-ozempic): Yes, for type 2 diabetes. Medicare drug coverage can pay for Ozempic when it's prescribed for its labeled uses, and your 2026 out-of-pocket drug costs are capped at $2,100. Ozempic isn't part of Medicare's $50-a-month GLP-1 Bridge for weight loss. Medicare's negotiated price, $274 for a 30-day supply, starts January 2027. Updated October 2026. - [Does Mounjaro make you pee more? Urination, thirst and dry mouth](https://longevityglp1.com/research/does-mounjaro-make-you-pee-more): Frequent urination isn’t a listed side effect of Mounjaro. The US label doesn’t name it, or dry mouth or thirst. It does warn that vomiting or diarrhea can dehydrate you and harm the kidneys. High blood sugar itself makes you pee more, so tell your prescriber about any change, especially peeing less. Updated October 2026. - [Does Wegovy make you pee more, or feel thirsty or sweaty?](https://longevityglp1.com/research/does-wegovy-make-you-pee-more): No. Frequent urination isn’t among Wegovy’s labeled side effects. The FDA prescribing information also leaves out dry mouth, thirst and sweating, but lists taste changes in 1.7% of adults. Its fluid warning points the other way: nausea, vomiting or diarrhea can dehydrate you and lead to kidney injury. Updated October 2026. - [Wegovy and tiredness: how common it is and what the label links it to](https://longevityglp1.com/research/does-wegovy-make-you-tired): Wegovy can make you tired. In its weight-loss trials, fatigue was reported by 11% of people on the 2.4 mg dose, compared with 5% on placebo. Headache (14% vs 10%) and dizziness (8% vs 4%) were also more common. The label doesn't say how long fatigue lasts, so tell your prescriber if it doesn't ease. Updated October 2026. - [Wegovy storage: fridge, room temperature and travel limits by device](https://longevityglp1.com/research/does-wegovy-need-to-be-refrigerated): Wegovy injections should be refrigerated, but the label allows a single-dose pen or syringe to be kept between 46°F and 86°F for up to 28 days before the cap is removed. The FlexTouch pen can stay at room temperature for 56 days after first use. Wegovy tablets don't need refrigeration. No form may be frozen. Updated October 2026. - [Does Zepbound cause constipation? Its stomach side effects, by dose](https://longevityglp1.com/research/does-zepbound-cause-constipation): Yes. In Zepbound’s weight-loss trials, constipation was reported by 17% of people on 5 mg, 14% on 10 mg and 11% on 15 mg, against 5% on placebo. Most stomach side effects showed up while the dose was being raised and eased over time. Updated October 2026. - [GLP-1 telehealth explained: routes, checks and FDA's red flags](https://longevityglp1.com/research/glp1-telehealth): GLP-1 telehealth means a licensed clinician evaluates you online, prescribes a GLP-1 if it's appropriate, and a pharmacy ships it. The drug can be an FDA-approved brand such as Wegovy, Zepbound or Foundayo, or a compounded version, which FDA doesn't review. Before paying, check FDA's red flags: no screening, "same as the brand" claims, an unclear pharmacy. Updated October 2026. - [Getting semaglutide online in 2026: compounded, brand or insurance](https://longevityglp1.com/research/how-to-get-compounded-semaglutide): To get semaglutide online you need a prescription from a licensed clinician. Compounded semaglutide comes from a state-licensed pharmacy filling that prescription for you, usually through a telehealth program. Brand Wegovy is also sold self-pay through NovoCare: the pill from $149 a month and pens at $349 after two intro months. Updated October 2026. - [How safe is compounded semaglutide? The risks FDA has documented](https://longevityglp1.com/research/is-compounded-semaglutide-safe): Compounded semaglutide isn't proven safe or unsafe as a category: it carries semaglutide's known risks, plus quality and dosing risks that FDA has documented and does not screen for. How safe your product is depends on the pharmacy that made it, the form, and how closely a prescriber follows you. Updated October 2026. - [How safe is compounded tirzepatide? Label risks plus compounding risks](https://longevityglp1.com/research/is-compounded-tirzepatide-safe): Compounded tirzepatide isn’t proven unsafe, but it isn’t checked either. It has all of tirzepatide’s known side effects, plus quality and dosing risks that FDA has documented and doesn’t screen for before it’s sold. Updated October 2026. - [Can you still get compounded tirzepatide? Where the rules stand](https://longevityglp1.com/research/is-compounded-tirzepatide-still-available): Compounded tirzepatide is still available in the U.S. as of this update, but only through patient-specific prescriptions filled by state-licensed pharmacies; the broad shortage-era copying ended in March 2025. Updated October 2026. - [How safe is Mounjaro? The label, the heart trial and who should avoid it](https://longevityglp1.com/research/is-mounjaro-safe): Mounjaro is FDA-approved and well studied for type 2 diabetes. Its main risks are digestive side effects, uncommon but serious problems such as pancreatitis and gallbladder disease, and a thyroid tumor warning based on rat studies. In a 13,299-person heart trial it was at least as safe for the heart as dulaglutide. Updated October 2026. - [Is Wegovy safe? What the FDA label and the trials show](https://longevityglp1.com/research/is-wegovy-safe): For most adults it's prescribed to, Wegovy has a well-studied safety record and mostly digestive side effects. It isn't safe for everyone. Its label carries a boxed warning about rodent thyroid tumors, rules it out with a history of medullary thyroid cancer or MEN 2, and lists rare serious risks like pancreatitis, gallbladder disease and kidney injury from dehydration. Updated October 2026. - [Does metformin slow aging? The studies, read carefully](https://longevityglp1.com/research/metformin-longevity): No trial has shown that metformin slows aging or extends life in people without diabetes. It is FDA-approved only for type 2 diabetes, so longevity use is off-label. Observational studies in people with diabetes point both ways, and two randomized trials in healthy older adults found it blunted gains from exercise. Updated October 2026. - [How to get a metformin prescription online, and what to expect](https://longevityglp1.com/research/metformin-prescription-online): You can get metformin through an online visit with a licensed prescriber, who will usually want a recent kidney test (eGFR) first. Because metformin is approved only for type 2 diabetes, using it for longevity, weight loss or PCOS is off-label and up to the prescriber. Online programs list it at $25–$162 a month. Updated October 2026. - [Metformin and tiredness: what's common, what's a warning sign](https://longevityglp1.com/research/metformin-tiredness): Metformin can make some people feel tired or weak: 9% reported weakness versus 6% on placebo in one trial. But unusual sleepiness together with muscle pain, trouble breathing or stomach pain can be a sign of lactic acidosis, a rare side effect that needs prompt medical care. Updated October 2026. - [Microdosing tirzepatide: the idea, the evidence gap and the risks](https://longevityglp1.com/research/microdose-tirzepatide): Microdosing tirzepatide means taking less than the doses tested for Zepbound, usually from a compounded vial. No clinical trial has studied it as of this update. Zepbound's label says 2.5 mg weekly is a starting dose, not approved for maintenance; the lowest tested maintenance dose, 5 mg, produced 15.0% average weight loss over 72 weeks. Updated October 2026. - [Microdosing GLP-1 drugs: the claims, the evidence and the risks](https://longevityglp1.com/research/microdosing-glp1): Microdosing a GLP-1 means taking less than the labeled doses of drugs like semaglutide or tirzepatide, usually from compounded vials. Right now no clinical trial has tested it for weight, longevity or wellness. The lowest doses on the Wegovy and Zepbound labels exist to start treatment, not to stay on. Updated October 2026. - [Bruising, itching and rash on Ozempic: what the label reports](https://longevityglp1.com/research/ozempic-bruising-itching-rash): Ozempic can leave a mark or redness where it's injected, but in its trials that happened to only 0.2% of people. Easy bruising elsewhere isn't on the label. A rash or hives across the body is different: those are reported after approval and can be part of an allergic reaction, which needs prompt care. Updated October 2026. - [Ozempic cost: cash price, insurance and assistance](https://longevityglp1.com/research/ozempic-cost-without-insurance): Without insurance, Ozempic costs $349 a month for the 0.25, 0.5 or 1 mg pen and $499 a month for the 2 mg pen through NovoCare Pharmacy. New patients pay $199 for each of their first two months at 0.25 or 0.5 mg, through December 2026. Ozempic pills start at $149 a month at current prices. Updated October 2026. - ["Ozempic face": facial changes after GLP-1 weight loss, explained](https://longevityglp1.com/research/ozempic-face): "Ozempic face" is an informal name for a hollower, looser-skinned face after major weight loss on GLP-1 drugs. It isn't on Ozempic's or Wegovy's label. Research points to weight-loss deflation rather than a semaglutide-specific effect: similar changes follow bariatric surgery, and imaging suggests about 7% midface volume loss per 22 pounds (10 kg) lost. Updated October 2026. - [How safe is Ozempic? What the label and two outcomes trials show](https://longevityglp1.com/research/ozempic-safety-and-side-effects): For adults with type 2 diabetes, Ozempic has a well-studied safety record. Its common side effects are digestive and mostly early. Its serious risks (pancreatitis, gallbladder disease, kidney injury from dehydration, eye complications in people with retinopathy, and a thyroid tumor warning based on rodent studies) are uncommon but worth knowing. It is not approved for weight loss. Updated October 2026. - [Ozempic and your stomach: diarrhea, nausea, constipation and burping](https://longevityglp1.com/research/ozempic-stomach-side-effects): Ozempic can cause diarrhea. In its placebo-controlled trials, 8.5–8.8% of people on Ozempic reported it, against 1.9% on placebo, and most cases came while the dose was being increased. Nausea was more common still, at up to 20%. The label gives no number of days these effects last. Updated October 2026. - [Peeing, thirst, dry mouth and taste changes on Ozempic](https://longevityglp1.com/research/ozempic-urination-thirst-dry-mouth): Peeing more isn't a listed side effect of Ozempic. The label's fluid warning runs the other way: vomiting or diarrhea can dehydrate you enough to injure the kidneys. Thirst and dry mouth aren't listed either. Taste changes are, at a low rate, and sweating appears only as a sign of low blood sugar. Updated October 2026. - [How people get a rapamycin prescription for longevity](https://longevityglp1.com/research/rapamycin-where-to-buy): Rapamycin (sirolimus) is prescription-only in the U.S., and using it for longevity is off-label. People get it through a doctor willing to prescribe it or a telehealth longevity program, filled as generic sirolimus or a compounded version. Programs charge $65–$207 a month as of this update. Sellers offering it without a prescription are not legitimate. Updated October 2026. - [Tirzepatide side effects: what the Zepbound and Mounjaro labels report](https://longevityglp1.com/research/tirzepatide-side-effects): Tirzepatide's most common side effects are digestive: nausea, diarrhea, vomiting and constipation. On Zepbound, nausea affected 25–29% against 8% on placebo; on Mounjaro, 12–18% against 4%. Both labels say these mostly happen while the dose is being raised and then decrease. Rarer serious risks include pancreatitis, gallbladder disease and kidney injury from dehydration. Updated October 2026. - [Wegovy cost: cash, insurance, Medicare and the savings card](https://longevityglp1.com/research/wegovy-cost): Without insurance, the Wegovy pen costs $349 a month through NovoCare Pharmacy at 0.25 to 2.4 mg, and $399 at 7.2 mg. New patients pay $199 for each of their first two months at 0.25 or 0.5 mg. Pills cost $149 to $299. Coverage plus the savings card can cut it to $25. Updated October 2026. - [Wegovy and your gallbladder: gallstones, inflammation and what to watch for](https://longevityglp1.com/research/wegovy-gallbladder): Yes, Wegovy can raise the risk of gallbladder problems. In adult weight-loss trials, gallstones were reported in 1.6% of people on the injection and 0.7% on placebo, and gallbladder inflammation in 0.6% versus 0.2%. The label says the higher rate held even after accounting for weight lost. It doesn't address people without a gallbladder. Updated October 2026. - [Wegovy nausea, constipation and diarrhea: how common, how long, and when to call](https://longevityglp1.com/research/wegovy-nausea): Nausea on Wegovy is most common while the dose is being increased, and for most people it eases. In trials, 44% of adults on Wegovy 2.4 mg had nausea, against 16% on placebo, and a typical bout lasted about 8 days. Call your prescriber if it won't go away, you can't keep fluids down, or you have severe pain. Updated October 2026. - [Wegovy side effects: what the label lists and how long they last](https://longevityglp1.com/research/wegovy-side-effects): The most common Wegovy side effects are digestive. In the 2.4 mg trials, nausea affected 44% of people (16% on placebo), diarrhea 30%, and vomiting and constipation 24% each. Most were mild to moderate and showed up around dose increases, then eased. Rare serious risks include pancreatitis, gallbladder disease, kidney injury from dehydration and allergic reactions. Updated October 2026. - [Wegovy and thyroid cancer: the boxed warning and the human evidence](https://longevityglp1.com/research/wegovy-thyroid-cancer): Nobody knows yet. Semaglutide, the drug in Wegovy, caused thyroid C-cell tumors in rodents, and the label says human relevance "has not been determined." A French study linked 1 to 3 years of GLP-1 use to more thyroid cancer; a larger Scandinavian study found no clear increase. People with a medullary thyroid carcinoma or MEN 2 history shouldn't use it. Updated October 2026. - [How to get compounded tirzepatide the legal way](https://longevityglp1.com/research/where-to-get-compounded-tirzepatide): You can get compounded tirzepatide only with a prescription from a licensed clinician, filled for you by a state-licensed compounding pharmacy. Most people go through a telehealth program; some local clinics prescribe it too. Since 2025, compounding is limited to individual prescriptions, so brand Zepbound from LillyDirect ($299 to $449 a month) is the main alternative. Updated October 2026. - [Is compounded semaglutide going away? What FDA has decided so far](https://longevityglp1.com/research/will-compounded-semaglutide-be-banned): Compounded semaglutide has not been banned in the United States as of this update, but the shortage-era allowance that made it widely available ended in 2025, and FDA has proposed a rule that would keep outsourcing facilities from compounding it. Updated October 2026. - [How much is Zepbound? Every price, with and without insurance](https://longevityglp1.com/research/zepbound-cost): Without insurance, Zepbound costs $299 a month at 2.5 mg, $399 at 5 mg and $449 at 7.5 to 15 mg through LillyDirect, if you refill within 45 days. With commercial coverage, Lilly’s savings card can cut that to $25. Eligible Medicare enrollees may pay $50 through the GLP-1 Bridge. Updated October 2026. - [Zepbound doses: the 2.5 mg start, the maintenance doses and the 15 mg maximum](https://longevityglp1.com/research/zepbound-dosage): The highest dose of Zepbound is 15 mg once a week, for every approved use. The label starts everyone at 2.5 mg, which is a starting dose only. Maintenance is 5, 10 or 15 mg for weight loss and 10 or 15 mg for obstructive sleep apnea. Your prescriber decides how and when your dose changes. Updated October 2026. - [Zepbound side effects: the full list, and how long they last](https://longevityglp1.com/research/zepbound-side-effects): Most Zepbound side effects are digestive, and they tend to show up when the dose goes up and fade over the following weeks. The label says most nausea, vomiting and diarrhea happened during dose escalation and decreased over time. There's no set number of days, and a dose increase can bring them back. Call your prescriber about anything severe. Updated October 2026. ## Matchups - [Is compounded semaglutide the same as Ozempic? A number travels with its context here: keep a Healthspan Score beside its four checks and the month it was read, a price beside its dose and plan length, and a trial figure beside the people it was measured in. Split any of those pairs and the number says something the page never did. Nothing here is medical advice; a prescriber decides what fits a patient.
