Our take on Pepti
Pepti shows every price up front, dose by dose, which few programs do. Compounded semaglutide starts at the lowest strength, and on a calendar-month basis that's $107 by our math; tirzepatide starts at $172. The card figure is a little lower because refills bill every 28 days, which is 13 charges a year, not 12. There's no membership fee, and the price covers the medication, physician review, refill management and free shipping.
The part to plan for is that the price follows your dose. Pick "I'm already on it" on the product page and the full ladder appears: each strength step costs more, and the top tirzepatide strength costs about twice the starting one. Pepti's FAQ says a physician-approved increase bills at the cost difference and a decrease doesn't change the price. So the first months cost the least, and year two at a maintenance dose will cost more than the first few months suggest. The compounded tirzepatide starting dose guide explains why plans start low, and the median across the 69 tirzepatide programs is $249.
Care is online and quick. You pay at checkout, a physician licensed in your state reviews the written assessment, usually within hours, and the medication charge is refunded automatically if they don't prescribe. Both GLP-1s need recent bloodwork: you can upload results from any lab at no charge or buy Pepti's at-home panel. Video visits aren't required in most cases, dose changes don't need an appointment, and you can message your physician any time. Its medical director and chief medical officer are named, though the partner physician network isn't.
Pepti names Olympia Pharmacy among its 503A partner pharmacies and says the pharmacy filling your order is printed on the label. The catalog is the big caveat. Most of its 59 treatments are peptides that US pharmacies can't lawfully compound, including BPC-157, TB-500, MOTS-c, ipamorelin, tesamorelin and injectable GHK-Cu, plus blends built on them. Stick to the GLP-1 plans. Read whether compounded semaglutide is safe, and compare programs on the compounded semaglutide lineup.

Prices are taken from Pepti's own site and current as of this update. Real monthly cost folds in required fees and turns four-week billing into calendar months; that's our math.
Compounded semaglutide skips FDA approval: no one at the FDA judges whether it is safe, whether it works, or how well it was made before a pharmacy sends it out.

What signing up looks like
Pick a dose and pay
Choose the starting dose, or the dose you're on now, and finish the online assessment. Payment is collected at checkout.
A physician reviews it
An independent physician licensed in your state reviews your answers, ID and labs, usually within hours. If they don't prescribe, the charge is refunded.
A 503A pharmacy compounds it
A partner pharmacy such as Olympia compounds your vial in one to three business days and ships it cold, free and unbranded.
Refills every 28 days
You get a text a few days before each refill bills. Your physician reviews the refill, and you can cancel in your account before renewal.
Who prescribes, and the pharmacy
Independent physicians licensed in your state, through a partner network Pepti doesn't name, review a written assessment; video isn't required in most cases. Its medical director and chief medical officer are named. Olympia Pharmacy is named among its 503A partner pharmacies, and your order's pharmacy appears on the label.
Where Pepti does well
- Every dose has a printed price, so you can see what the maintenance dose will cost before you start.
- No membership fee: medication, physician review, refill management and shipping are in one price.
- Names Olympia Pharmacy among its 503A partners and says your pharmacy is printed on every label.
- The medication charge is refunded automatically if the physician decides not to prescribe.
- You can upload bloodwork from any lab for free instead of buying a new panel.
What to check first
- Refills bill every 28 days, which is 13 charges a year. Budget on the calendar-month figure, not the card price.
- The price rises with each dose step. Check the dose ladder and price your likely maintenance dose before you start.
- Payment is taken at checkout, before review. Expect a refund to take up to two weeks if you aren't prescribed.
- Most of the catalog is peptides that can't be lawfully compounded, like BPC-157. Stick to the GLP-1 plans.
- GLP-1s need recent bloodwork. Upload labs you already have to avoid paying for the at-home panel.
It fits you if
You want to see every dose's price before you commit, like a named pharmacy, and are comfortable with an online-only physician review and 28-day billing.
Look elsewhere if
You want one price at every dose, monthly rather than 28-day billing, or a program that sells only drugs pharmacies can lawfully compound.

Questions about Pepti
- How much does Pepti semaglutide cost?
- At the starting dose it's about $107 a calendar month by our math, billed every 28 days. Higher doses cost more, and the product page lists each one. Tirzepatide starts at about $172 a month.
- Does Pepti charge more when my dose goes up?
- Yes. Its FAQ says an approved increase bills at the cost difference for the higher strength, and a dose reduction doesn't change the price.
- Do I need labs for Pepti's GLP-1s?
- Yes, recent bloodwork is required for semaglutide and tirzepatide. You can upload results from any lab at no charge or add Pepti's at-home panel.
- Which pharmacy fills Pepti orders?
- Pepti names Olympia Pharmacy among its partner 503A compounding pharmacies. The pharmacy for your order depends on your state and medication and is printed on the label.
- How do I cancel Pepti?
- Cancel in your account before the renewal date and the next refill won't bill. There's no fee to cancel, but supply already shipped isn't refunded.

None of the GLP-1 labels tells you whether you can drink. Several of their warnings still overlap with alcohol, and metformin's label is far more direct. Here's what each one says.
- Drug guideSemaglutide: how it works and the products that contain it
Semaglutide copies GLP-1, a hormone your gut releases after eating. It raises insulin only when blood sugar is high, lowers glucagon, slows stomach emptying and reduces appetite through the brain. That is why it lowers blood sugar and body weight. It is sold as Ozempic and Rybelsus for diabetes and as Wegovy for weight.
