How this lineup is built
Every program here clears one test on top of its score: its month-to-month price at the starting dose, with any required membership added in, comes to less than $250. The order below that line is the same Healthspan Score order as the full tirzepatide lineup, so a program sits in the same place relative to its neighbors on both pages.
Price is one of four parts of that score, worth 35 of 100 points (how we score). On tirzepatide's band, a plate at $140 or less earns all 35, which is why so many plates here carry a full or nearly full Price chip. The other 65 points decide the order among plans that all cost about the same.
Compounded tirzepatide isn't FDA-approved, and the FDA doesn't check a compounded version's safety, effectiveness or quality before it ships. A low price doesn't change that, so the Pharmacy chip matters as much here as anywhere.
Under $250 is the start of the comparison, not the end
Once every plan clears the same price line, the plate stops telling programs apart. Two plans a few dollars apart can differ a lot in who prescribes, who fills the vial and what happens when your dose goes up.
So read the chips before the plate. A full Price chip next to a Care chip of 5 or 10 usually means a lean plan: an intake, a prescription and little stated follow-up. A program a little higher on the plate with stronger Care and Pharmacy chips may cost you less trouble by month three.
The chip line under each name is a shortcut, too. Phrases like “Same price every dose”, “Pay as you go” or “Yearly membership” describe how that program bills, and each one changes what the plate means for you later.
Look at the state line on each review before anything else if you live outside the big states. A few programs that clear the price line serve only a couple of states or publish no state list at all.
What sits inside a plate under the line
- Required membership, spread by the month. A plan that charges a yearly membership has that fee divided by twelve and added to its monthly price. So the plate can read above the drug price the program advertises, and that's on purpose.
- Four-week billing, restated. A plan billed every four weeks charges 13 times a year. Its plate is that charge times 13, divided by 12, so it may read a few dollars above the program's own figure.
- The month-to-month price only. Many programs here advertise a lower “starting at” figure that turns out to be the rate on a 6- or 12-month plan paid up front. The plate ignores those rates, so a plan doesn't slip under the line just by asking you to prepay.
- No first-month promos. A discounted first order or a struck-through intro price doesn't count. The plate is what month two costs at the starting dose.
Why some plans cross $250 after month one
Plates on this page run $85 to $249 a month, and the middle one is $199. At that middle plate, a year of the starting dose comes to about $2,388 by our math.
The catch is the word “starting.” Zepbound's label starts at 2.5 mg a week for four weeks and calls that dose a starting dose only, so most people move up. A plan that prices by dose can clear the line in month one and cross it once the dose climbs, and at least one program here lists a higher price at every milligram step.
Programs that charge the same at every dose avoid that jump. If you expect to stay on tirzepatide for a year, a flat plan near the top of this price range can cost less over twelve months than a dose-priced plan near the bottom.
For scale, brand Zepbound through the telehealth programs we track runs $299–$1,828 a month, and the Zepbound guide lists LillyDirect's own self-pay vial prices by dose. Across those programs, while 69 programs list compounded tirzepatide at $85–$433. The compounded tirzepatide cost guide walks through the later doses.
Prepaying is the other way programs get lower. Some plans here cost noticeably less per month on a 6- or 12-month term, charged at once. That only saves money if you stay, so ask what you get back if you stop early before you take the long plan.
How to read this lineup
- Every plate here is under $250 a month at the starting dose, so compare the four chips first and the plate second.
- The score and its parts come straight from the full tirzepatide lineup. Nothing is rescored for this page.
- A Partner tag marks a program that may pay us if you start care through our link. It changes the button, never the place in the list, and the labeled Partner box above the list stands apart from the order.
Five questions that show what the low price leaves out
- What will I pay at the second and third dose steps, not just the first?
- Are labs required, and are they inside this price or billed on top?
- Does the vial contain anything besides tirzepatide, such as B12 or glycine, and is that in the price?
- If my medication ships three months at a time, do I owe for the whole shipment if I cancel after one month?
- Is the membership refundable, and is it charged up front or spread over the year?
Questions about this lineup
- Can you get compounded tirzepatide for under $250 a month?
- Yes. The 36 programs here charge $85 to $249 a month at the starting dose with required fees added in. Ask what later doses cost, because some plans charge more as the dose rises and can cross $250 after the first month.
- Why is a program's advertised price lower than its plate?
- Usually for one of three reasons: the advertised figure is a 6- or 12-month prepaid rate, a membership fee is billed separately, or the plan bills every four weeks. The plate adds required fees and restates the price as a calendar month, so you can line programs up fairly.
- Is a lower-cost tirzepatide plan a worse plan?
- Not by itself. Several plans here earn strong Care and Pharmacy chips. What a low price sometimes leaves out is follow-up, labs, or a named pharmacy, and those chips show it on each card before you open the review.
- How does compounded tirzepatide under $250 compare with brand Zepbound?
- Brand Zepbound through telehealth programs runs $299–$1,828 a month, so every plate here sits below the brand's range. Zepbound is FDA-approved and made by Eli Lilly; compounded tirzepatide is made by a pharmacy without FDA approval. The compounded tirzepatide cost guide covers how prices change with the dose.
The Healthspan Score
Every provider gets a 0 to 100 Healthspan Score for each drug it prescribes, built from four checks: what a new patient pays per month at the starting dose (35), how care works (25), whether the pharmacy is named (25) and how clearly prices and terms are shown (15).
