Our take on Lttl
Lttl sells compounded semaglutide and tirzepatide on a monthly auto-refill, and each refill is a four-week supply. That's 13 refills a year, so the real monthly cost is about $107.25 for semaglutide and $161.42 for tirzepatide by our math. Those are low figures for this category, and Lttl says the price is the same at every dose, which is the part that matters past month three. The "starting at" figure on the product pages is the 12-month plan, though. Budget by the auto-refill price unless you're ready to prepay. The compounded semaglutide guide covers how the drug compares with Wegovy.
Two things deserve a careful read before you check out. First, a first-month discount is applied automatically, and the discounted first charge isn't shown until checkout, so month two is the number to plan around. Second, the refund rules are tight. Once your intake is submitted the order counts as processed and isn't refundable, unless a clinician finds you aren't eligible or there's a billing mistake. Refund requests have to go in writing within 28 days. Canceling stops future charges, and you do it by emailing the care team.
Care runs through independent networks of U.S.-licensed clinicians, which Lttl doesn't name. The visit is fully online, and your clinician raises the dose gradually with check-ins along the way. Lttl does name its two partner compounding pharmacies in its terms, which many programs skip. If you want a smaller dose, there's a microdose line: tirzepatide at $149 a month after a first month of $119. The GLP-1 microdosing guide explains what is and isn't known about lower doses.
Lttl also sells NAD+, sermorelin and glutathione, and a dissolving semaglutide tablet for people who'd rather not inject. Its "coming soon" peptide page is worth ignoring: it lists compounds FDA hasn't cleared for compounding. To see how Lttl stacks up on price, care and pharmacy, the compounded semaglutide lineup scores every program the same way.

Prices are taken from Lttl's own site and current as of this update. Real monthly cost folds in required fees and turns four-week billing into calendar months; that's our math.
Compounded semaglutide skips FDA approval: no one at the FDA judges whether it is safe, whether it works, or how well it was made before a pharmacy sends it out.

What signing up looks like
Take the online assessment
A two-minute eligibility quiz, then a health history. Plan prices are on the plans page before you start.
A clinician reviews it
A U.S.-licensed clinician in your state reviews your answers online and, if appropriate, prescribes a starting dose.
A partner pharmacy fills it
One of Lttl's two partner compounding pharmacies prepares the order, and it ships in two to five business days.
Refills arrive every four weeks
The auto-refill sends a four-week supply each cycle. Your clinician steps the dose up with check-ins, at the same price.
Who prescribes, and the pharmacy
Prescriptions come from independent networks of U.S.-licensed clinicians that Lttl doesn't name; its medical advisory board reviews protocols but doesn't prescribe. The visit is fully online. Lttl's terms name two partner compounding pharmacies, Precision Medicine and Red Rock Compounding Pharmacy, describe them as state-licensed, and say a prescription may be moved between pharmacies for availability or pricing reasons.
Where Lttl does well
- Says the price stays the same at every dose, so a higher maintenance dose doesn't raise the bill.
- Prints every plan length and price for each product on one plans page.
- Names its two partner compounding pharmacies in its terms of service.
- Low auto-refill prices for both compounded semaglutide and tirzepatide.
- Semaglutide also comes as a dissolving tablet, and a microdose line sits alongside the full doses.
What to check first
- Each refill is a four-week supply, so 13 charges land a year. Budget by the real monthly cost.
- Product pages lead with the 12-month price. Compare on the monthly auto-refill price instead.
- Orders aren't refundable once your intake is in, except if you're found ineligible. Decide before you submit.
- The prescribing clinicians are never named. Ask the care team who will review your intake.
- Its peptide waitlist lists compounds FDA hasn't cleared for compounding. Stick to its GLP-1 programs.
It fits you if
You want a low monthly price on compounded semaglutide or tirzepatide that doesn't climb with your dose, and you're happy with a fully online visit.
Look elsewhere if
You want a refund window after you submit your intake, a named prescriber, or labs built into the plan.

Questions about Lttl
- How much does Lttl cost a month?
- Compounded semaglutide comes to about $107.25 a month and tirzepatide about $161.42 on the auto-refill, counting 13 four-week refills a year. The price is the same at every dose.
- Does Lttl's price go up as my dose increases?
- No. Lttl says its plans cost the same at every dose, so moving up to a maintenance dose doesn't change the monthly price.
- Can I get a refund from Lttl?
- Only in narrow cases: if a clinician finds you aren't eligible, or there's a billing error. Once your intake is submitted, the order is otherwise non-refundable.

Semaglutide copies GLP-1, a hormone your gut releases after eating. It raises insulin only when blood sugar is high, lowers glucagon, slows stomach emptying and reduces appetite through the brain. That is why it lowers blood sugar and body weight. It is sold as Ozempic and Rybelsus for diabetes and as Wegovy for weight.
