How this lineup is built
Most rows are compounded semaglutide or tirzepatide in a lozenge, drop, capsule or tablet. The rest are brand pills: the Wegovy pill, which the FDA approved in December 2025, and Foundayo, approved in April 2026. They share a lineup because you're asking one question: which pill program, at what monthly price.
Nova has the highest Healthspan Score here, 75 out of 100. The score has four parts: price 35, care 25, pharmacy 25 and clarity 15 (how we score). Here, a pill plan at $140 a month or less earns the full 35 price points. One at $350 or more earns zero, and one near $245 earns about half. The band is fixed for the lineup, so a rival's price change never moves anyone else's points.
Pick the form first, then compare the price
A pill lineup mixes products that only share a drug family. A brand tablet, a compounded lozenge and a compounded drop are made differently and come with different evidence, so the first filter is the form you want, not the plate.
Once you've picked a form, compare score and plate as you would on any lineup. The chip row helps here: a strong Pharmacy chip means the provider has said who makes the product, which matters more when it isn't a brand.
A provider that sells only a brand pill earns two of the pharmacy checks automatically once the product is named. Compounded programs have to earn those points on their own pages.
Already on an injection and thinking of switching? Pills and shots aren't dosed the same way, so ask the program how it would choose your pill dose. That conversion is the prescriber's call, not a chart's.
Last, check where the pill is made. A brand tablet comes from its maker; a compounded one comes from a pharmacy, and the Pharmacy chip tells you whether the provider says which.
What the four checks look for in a pill program
- Price (35). The monthly price at the starting dose, membership included. For a brand pill that's the price the program charges you, not the maker's list price.
- Care (25). Five 5-point checks: a named prescriber or medical group, a clinician's review of your intake, labs, a stated follow-up rhythm, and dose changes without a new consult fee.
- Why follow-up matters here. With a daily pill, a stated check-in rhythm is how you'll find out whether the dose suits you, so the Care chip is worth a second look.
- Pharmacy (25). Up to 25 points for telling you where the pill comes from: the pharmacy's name (10), its state or 503A/503B status (5), a clear brand-or-compounded label (5) and no claim that a compounded pill is FDA-approved (5).
- Clarity (15). Up to 15 for showing a price before the questions start, publishing cancellation terms and listing the states served.
Pill prices over a full year
Plates on this lineup run $127 to $369 a month, so the spread over a year is about $2,904 by our math. At the middle plate, $279, a year costs about $3,348.
Site-wide, 23 programs offer compounded semaglutide in a form you swallow, at $127–$369 a month, and 16 offer oral compounded tirzepatide at $189–$399.
Within this lineup's band, ten dollars off the monthly price is worth about 1.7 price points.
Brand pill prices on this lineup are what each program charges, and they can move when the maker changes its self-pay prices. A compounded pill's price is the provider's own and changes when its pricing page does.
Four-week billing is folded in the same way as on the injection lineups: 13 charges a year, restated as a calendar month, so a plate can sit a little above the provider's printed figure.
Pill doses are larger in milligrams than shot doses because less of a swallowed dose reaches the blood. That doesn't by itself make a pill cost more, but it's why the dose printed next to a price matters.
How to read this lineup
- Each row pairs a Healthspan Score with the starting-dose monthly price, so a lozenge plan and a brand pill sit on the same two numbers.
- Check the line under each name for the form: a dissolving tablet, a drop and a capsule are different products, even with the same molecule inside.
- Four-week billing is restated as a calendar month. That keeps a provider billing 13 times a year from looking less expensive than it is.
- Partner rows may pay us when you start care. They show a tag and a filled button and sit where their score puts them; an exact tie is the one case where a partner goes first. The Partner box above the list is separate from the scored order.
- “No monthly price to compare” (10 here) marks a provider whose pages don't give one month-to-month starting price. That row's price part counts as zero.
What to ask a pill program
- Is this a brand tablet or a compounded product, and what exactly is the form?
- Which pharmacy or maker supplies it, and does the label list every ingredient?
- Does the price rise as the dose goes up?
- Can you switch to an injection on the same plan if the pill doesn't suit you?
- Is there a membership in addition to the pill price, and what does it cover?
- What does canceling cost, and when does the next charge land?
Questions about this lineup
- What does a GLP-1 pill program cost a month?
- At the starting dose, the 30 programs here with a month-to-month price charge $127 to $369, with a midpoint of $279. That includes any required membership. Prices often change with the dose, so check which dose a price belongs to.
- Are GLP-1 drops and lozenges the same as the Wegovy pill?
- No. The Wegovy pill is an FDA-approved tablet made by Novo Nordisk and tested in its own trials. A compounded semaglutide drop or lozenge is made by a pharmacy, isn't FDA-approved, and has no trial data of its own.
- Why are brand and compounded pills on one lineup?
- Because they answer the same question: a GLP-1 you swallow, at a monthly price. The Healthspan Score treats both the same way, and the pharmacy check rewards a provider that says plainly which kind you're getting.
- Do GLP-1 pills cost less than shots?
- Not reliably. Across the site, compounded semaglutide in a form you swallow lists at $127–$369 a month, and the injectable runs $75–$349. Which costs less for you depends on the provider and the dose.
The Healthspan Score
Every provider gets a 0 to 100 Healthspan Score for each drug it prescribes, built from four checks: what a new patient pays per month at the starting dose (35), how care works (25), whether the pharmacy is named (25) and how clearly prices and terms are shown (15).
