Our take on Willow
Willow charges $299 a month for compounded semaglutide and $399 for compounded tirzepatide, and each comes as a weekly injection or a daily tablet at the same price. That one subscription covers the medication, the physician's evaluation, unlimited messaging and two-day shipping, with no membership on top. There's no cheaper multi-month plan either, so the figure on the product page is what you pay every month. Over a year, that's twelve equal charges with nothing extra to add. The compounded semaglutide guide explains what the drug is.
Care is asynchronous. You fill out a detailed health history online, and a US-licensed physician from Willow Health Services Medical of California, an independent medical group, reviews it, usually within 24 hours. There's no phone or video call. After that, you message your doctor through the portal whenever you need to, and Willow says you usually keep the same physician. The site also publishes profiles of its doctors, so you can see who might be treating you.
Two limits are worth knowing before you start. Willow doesn't accept people with diabetes, though prediabetes and high blood pressure are fine. And its two state lists differ: the FAQ adds Wyoming and the help center adds Puerto Rico, so only the 35 states on both lists are certain. Confirm yours on the first screen of sign-up. The tirzepatide tablet is a compounded product, not a brand-name pill.
Willow says its medications come from state-licensed compounding pharmacies that follow USP standards, but it doesn't name them. You can send your prescription to a pharmacy of your choice instead, though a monthly platform fee applies then. To cancel, message the care team at least two business days before your next processing date. If you cancel before the prescription reaches the pharmacy, you get a full refund; once the medication is compounded, it can't be returned. The compounded semaglutide lineup sorts Willow with other programs by Healthspan Score, and the GLP-1 pill lineup covers its tablets.

Prices are taken from Willow's own site and current as of this update. Real monthly cost folds in required fees and turns four-week billing into calendar months; that's our math.
Compounded semaglutide skips FDA approval: no one at the FDA judges whether it is safe, whether it works, or how well it was made before a pharmacy sends it out.

What signing up looks like
Check your eligibility
A quick assessment comes first, then your health history and your choice of drug and form.
A physician reviews it
A US-licensed physician from Willow's medical group reviews your history, usually within 24 hours, and writes a plan if appropriate.
A pharmacy prepares it
After an identity check, the prescription goes to a partner compounding pharmacy, which takes two to four business days.
Ships, then message anytime
Your order ships with tracking in about two days, and you message your doctor through the portal for any changes.
Who prescribes, and the pharmacy
US-licensed physicians from Willow Health Services Medical of California, an independent medical group, review your online health history and write your plan, with no phone or video visit; afterward you message the same physician where possible. Willow says state-licensed compounding pharmacies that meet USP standards prepare its medications, but its terms, privacy policy, FAQ and help center don't name them.
Where Willow does well
- One flat monthly price covers medication, physician care, messaging and two-day shipping.
- Semaglutide and tirzepatide each come as a shot or a daily tablet at the same price.
- Publishes profiles of its physicians, and you usually keep the same doctor.
- Full refund if you cancel before the prescription goes to the pharmacy.
- Its timeline puts most patients' medication at the door within a week of approval.
What to check first
- There's no multi-month plan to lower the monthly price. Budget for the full figure every month.
- Not open to people with diabetes. Pick a program that accepts diabetes if that applies to you.
- Its FAQ and help center list different states; only the 35 on both are certain. Confirm yours at the start of sign-up.
- The pharmacy isn't named. Ask the care team which pharmacy fills your order before you start.
- No refund once the medication is compounded. Cancel at least two business days before processing.
It fits you if
You want compounded semaglutide or tirzepatide at one simple monthly price, as a shot or a tablet, with a physician you can message rather than meet on video.
Look elsewhere if
You have diabetes, you want a live visit, you live outside its states, or you want a lower monthly price for committing to several months.

- ResearchWhat’s the highest dose of compounded semaglutide? There’s no label, only brand limits
A maximum dose is something FDA writes into a drug’s label, and compounded semaglutide doesn’t have one. The approved semaglutide injections do. Here are their limits, what FDA has seen when compounded doses go past them, and what to ask about yours.
- Drug guideSemaglutide: how it works and the products that contain it
Semaglutide copies GLP-1, a hormone your gut releases after eating. It raises insulin only when blood sugar is high, lowers glucagon, slows stomach emptying and reduces appetite through the brain. That is why it lowers blood sugar and body weight. It is sold as Ozempic and Rybelsus for diabetes and as Wegovy for weight.

The name on the vial is the same molecule. Almost everything else differs: who checks it, what else is in it, how it's dosed and what evidence stands behind it. Here is each difference, with current prices for both.
