Question

Ozempic storage: the fridge before first use, then 56 days

The answer changes once you've used the pen. Here is the label's rule before and after first use, the different rule for the prefilled syringe and the tablets, and what freezing or heat means for your medicine.

The short answer

Before first use, yes. Keep new Ozempic pens in the fridge at 36°F to 46°F, away from the cooling element, and never frozen. After the first injection, a pen keeps for 56 days at 59°F to 86°F or in the fridge, cap on, out of heat and sunlight. Then it's thrown away.

Key takeaways

  • Before first use, keep Ozempic pens in the fridge at 36°F to 46°F, away from the cooling element.
  • After first use, a pen can be kept for 56 days at 59°F to 86°F or in the fridge, with the cap on, then thrown away.
  • The single-dose prefilled syringe stays refrigerated but may be kept at 46°F to 86°F for up to 28 days if needed.
  • Never freeze Ozempic, and don't use a pen or syringe that has been frozen.
  • Ozempic tablets don't go in the fridge; they're stored at room temperature in their original bottle.

Yes, until you start using it. An unused Ozempic pen belongs in the refrigerator, but once you've taken your first dose, the label lets the pen live at room temperature or in the fridge for 56 days [1]. The syringe and the tablets each have their own rule, covered below.

The rule in one table

Before first use: the fridge, away from the cooling element

Prior to first use, the OZEMPIC pen should be stored in a refrigerator between 2°C to 8°C (36°F to 46°F). Do not store in the freezer or directly adjacent to the refrigerator cooling element.[1]

The label adds that Ozempic “must be kept in the original carton until time of administration,” which protects it from light. A pen kept in the fridge this way is good until the expiration date printed on its label [1].

One thing the label doesn't give an unused pen is a room-temperature allowance. Wegovy's single-dose pens can sit out for up to 28 days before the cap comes off; the Ozempic pen's label sets no such window before first use. If a new pen was left out, ask your pharmacist before you use it.

After first use: 56 days, at room temperature or in the fridge

After first use of the OZEMPIC pen, the pen can be stored for 56 days at controlled room temperature 15°C to 30°C (59°F to 86°F) or in a refrigerator 2°C to 8°C (36°F to 46°F).[1]

So after your first dose, you pick: room temperature up to 86°F or the fridge are both allowed. The label adds three conditions: “Keep the pen cap on when not in use,” protect it “from excessive heat and sunlight,” and don't freeze it [1].

The 56 days run from first use, wherever you keep it. The Medication Guide is firm: “The OZEMPIC pen you are using should be disposed of (thrown away) after 56 days, even if it still has OZEMPIC left in it” [1]. It suggests writing the date on your calendar. Each pen holds several weekly doses, so the limit matters most when a partly used pen is left over.

The prefilled syringe follows a 28-day rule instead

Ozempic also comes in single-dose prefilled syringes at 0.25 mg, 0.5 mg and 1 mg, the doses explained in how Ozempic is taken. These stay in the fridge, but “if needed, the syringe can be kept from 8°C to 30°C (46°F to 86°F) up to 28 days” [1]. Keep them in the carton to protect them from light.

The syringe instructions ask you to note the date it came out of the fridge and to throw it away if it isn't used within 28 days. Each syringe is single use and is discarded after its one dose.

Freezing ruins it; heat and sunlight damage it

Freezing is the one hard stop. The label says: “Do not freeze OZEMPIC and do not use OZEMPIC if it has been frozen” [1]. That applies even if it has thawed and looks normal. Keeping pens away from the fridge's cooling element is how the label tries to prevent it.

Heat is the other risk. The label's ceiling for an in-use pen is 86°F, and it tells you to keep Ozempic “away from heat and out of the light” [1]. If a pen froze or got hotter than that, ask your pharmacist before using it and about a replacement.

Traveling with Ozempic: plan around 86°F

For a trip, the label's temperatures are the rulebook. A pen you've already started can travel at room temperature, as long as it stays between 59°F and 86°F, away from direct sun, and never freezes [1]. A pen you haven't started yet is labeled for the fridge only, so plan to keep it cold, at 36°F to 46°F.

If you're getting Ozempic by mail or telehealth, how to get Ozempic covers the pharmacy routes. Your pharmacist can help you plan how much to bring and how to keep it in range for the length of your trip. The syringe's 28-day allowance and the tablets' room-temperature rule make those two forms simpler to travel with.

Ozempic tablets stay at room temperature

The daily Ozempic tablet is a different product with a simpler rule. It's stored at 68°F to 77°F, with short trips between 59°F and 86°F allowed, in its original closed bottle and away from moisture [2]. The label says not to move the tablets into another container, such as a pill organizer. Both forms are the same molecule, described on our semaglutide page.

Compounded semaglutide doesn't follow any of these labels. Its storage is set by the pharmacy that makes it, and compounded drugs aren't FDA-approved or checked by FDA for safety, effectiveness or quality before they ship. Its rules are covered in compounded semaglutide storage. What Ozempic costs if a pen is lost to heat or freezing is in Ozempic cost without insurance.

Frequently asked

How long can Ozempic be out of the fridge?
Once you've used a pen, up to 56 days at 59°F to 86°F, then it's thrown away. A prefilled syringe can be kept at 46°F to 86°F for up to 28 days if needed. The label gives an unused pen no time outside the fridge.
Does Ozempic need to be refrigerated after opening?
No. After first use, the label lets you keep the pen at room temperature up to 86°F or in the fridge for 56 days. Keep the cap on and keep it out of heat and sunlight.
What if Ozempic freezes?
Don't use it. The label says not to use Ozempic that has been frozen, even if it has thawed. Ask your pharmacist about a replacement.
Can Ozempic go back in the fridge?
After first use, the label allows either room temperature or the fridge for the 56 days, so an in-use pen can be kept in the fridge. The 56-day limit still counts from first use.
Does compounded semaglutide follow the same rule?
No. Compounded semaglutide isn't Ozempic, and its storage is set by the pharmacy that makes it. Follow the pharmacy's label and ask the pharmacy if anything is unclear.
