Wegovy nausea, constipation and diarrhea: how common, how long, and when to call

Stomach side effects are the most common thing people feel on Wegovy. They're most frequent while the dose is climbing, and individual bouts of nausea in the trials were usually short. Here are the numbers, how long each symptom tended to last, and the patterns that mean something more serious.

The short answer

Nausea on Wegovy is most common while the dose is being increased, and for most people it eases. In trials, 44% of adults on Wegovy 2.4 mg had nausea, against 16% on placebo, and a typical bout lasted about 8 days. Call your prescriber if it won't go away, you can't keep fluids down, or you have severe pain.

Key takeaways

  • 73% of adults on Wegovy 2.4 mg reported at least one digestive side effect in weight trials, vs 47% on placebo.
  • Nausea 44%, diarrhea 30%, vomiting 24% and constipation 24%, vs 16%, 16%, 6% and 11% on placebo.
  • The label says these reactions "were most frequently reported during dosage escalation." In a pooled analysis, a typical bout of nausea lasted a median of 8 days; constipation lasted longer, a median of 47 days.
  • Severe digestive reactions affected 4.1% vs 0.9%, and 4.3% stopped Wegovy because of them.
  • Warning signs: severe stomach pain that won't go away, sometimes felt in the back; upper-belly pain with fever or yellowing; vomiting or diarrhea that won't stop.

If your stomach is unhappy on Wegovy, you're in the majority. Nearly three in four adults in its weight trials reported a digestive side effect, against about half on placebo [1]. Most of it is mild and temporary. A small part of it isn't, and the difference is worth knowing. The drug behind Wegovy is covered in semaglutide.

Wegovy nausea is most common while the dose is rising

The label doesn't give a duration, but it does say when digestive side effects cluster: “These reactions were most frequently reported during dosage escalation” [1]. In the STEP 1 trial, that escalation phase lasted the first 16 weeks before people reached the 2.4 mg maintenance dose [3]. Your own schedule is set by your prescriber.

A pooled analysis of the STEP 1 to 3 trials looked closer. In the semaglutide 2.4 mg group, the share of people with nausea, diarrhea or vomiting peaked at about week 20 and fell after that. Individual bouts were short: the median nausea episode lasted 8 days, diarrhea 3 days and vomiting 2 days, similar to how long the same symptoms lasted on placebo [2]. The authors described most episodes as mild to moderate and transient. Two of them are Novo Nordisk employees, and the trials were Novo's.

Median length of one episode in the pooled STEP 1–3 trials, semaglutide 2.4 mg
Nausea
8 days[2]
Diarrhea
3 days[2]
Vomiting
2 days[2]
Constipation
47 days[2]

A median is the middle value, so half of episodes were shorter and half longer. And these are single episodes, not a whole course of treatment: the analysis counted episodes, so one person could have several.

Wegovy slows your stomach, which is part of how it works

The label states it plainly: “WEGOVY delays gastric emptying” [1]. Food stays in your stomach longer, and that's one reason fullness and queasiness are so common. Semaglutide also acts on appetite, which is how it reduces hunger. The two effects arrive together, which is why the label builds its dose increases around the stomach.

Wegovy prescribing information, Table 3 and the digestive side-effect section (2,116 people on Wegovy, 1,261 on placebo) [1]. On the tablets, severe digestive side effects were reported by 2% vs 0% on placebo.

The label also lists hemorrhoids and hiccups as more common on Wegovy, without rates. Teens had a rougher time than adults: in the trial of children 12 and older, 62% on Wegovy reported a digestive side effect against 42% on placebo, with vomiting in 36% versus 10% [1]. The brand and its approved uses are covered in Wegovy. Zepbound has a similar pattern with its own numbers, covered in does Zepbound cause constipation.

About 1 in 4 adults had constipation, and it lasted longer than nausea

Constipation was reported by 24% on Wegovy and 11% on placebo [1]. It behaves differently from nausea. In the pooled analysis, a typical constipation episode lasted a median of 47 days, against 35 days on placebo, and the share of people affected leveled off around week 10 rather than falling [2].

Rarely, constipation becomes serious. After approval, reports came in of “ileus, intestinal obstruction, severe constipation including fecal impaction” with semaglutide [1]. These come from voluntary reports, so there's no rate. If constipation is severe, or comes with vomiting, a swollen belly or stomach pain, call your prescriber promptly.

Heartburn, gas and bloating are common too

Reflux (5% vs 3%), gas (6% vs 4%), bloating (7% vs 5%) and burping (7% vs under 1%) were all more common on Wegovy [1]. Burping stands out because it almost never happened on placebo. The label doesn't say how long heartburn lasts, so if it's sticking around, that's a question for your prescriber.

For most people it gets better, but not for everyone

Most people stay on Wegovy through the stomach phase. But 4.3% of adults in the weight trials stopped because of digestive side effects, against 0.7% on placebo [1]. In the pooled analysis, 99.5% of digestive events on semaglutide were non-serious and 98.1% were mild to moderate [2].

One finding may reassure you if you're worried that feeling sick is the point. The same analysis found weight loss was similar in people with and without digestive side effects, and less than one percentage point of the extra weight loss was explained by them [2]. Nausea isn't how Wegovy works.

When nausea is a warning sign

Severe, lasting stomach pain, vomiting you can't stop, or signs of dehydration need prompt medical attention. Each one connects to a warning on the label [1].

What you can ask your prescriber

The dose schedule exists to limit these effects, so ask before changing anything yourself. The label leaves dose changes to your prescriber. Good questions to bring:

  • Is what I'm feeling typical for this point in treatment?
  • Should we change how my dose is going up?
  • Is there anything you'd recommend for the nausea or constipation, given my other medicines?
  • Which symptoms should make me call you right away?

The Medication Guide adds one line of its own: “It is important for you to drink fluids to help reduce your chance of dehydration. Tell your healthcare provider right away if you have nausea, vomiting, or diarrhea that does not go away” [1]. You can also report side effects to FDA's MedWatch program [4].

Some programs sell lower-than-label doses of GLP-1s to reduce nausea. No trial supports that approach; the evidence gap is in microdosing GLP-1s. The full Wegovy side-effect list is in Wegovy side effects, and the tablet version is covered in oral semaglutide.

Frequently asked

Why does Wegovy make me nauseous after a dose increase?
Wegovy slows how fast your stomach empties, and the label says digestive side effects were most often reported during dose escalation.
Does Wegovy nausea come back at each new dose?
It can. The label says digestive reactions were most frequent during dose escalation, which covers each increase. It doesn't say every increase will bring nausea back.
Does Wegovy cause diarrhea or constipation more?
Diarrhea was a little more common: 30% vs 24% for constipation in the label's weight trials. Constipation episodes tended to last longer, though, a median of 47 days against 3 days for diarrhea in a pooled analysis.
Does heartburn on Wegovy go away?
The label doesn't give a duration for reflux, which affected 5% on Wegovy vs 3% on placebo. If it lasts or keeps you up at night, ask your prescriber.
Is nausea a sign Wegovy is working?
No. In a pooled analysis of three trials, weight loss was similar in people with and without digestive side effects. Nausea isn't required for Wegovy to work.
