Wegovy pill or Wegovy shot: cost, results and the trade-offs

Same molecule, same company, two very different routines. The pill is cheaper on NovoCare's cash prices and did nearly as well in its trial. The weekly pen covers more people and goes to a higher dose.

The short answer

Yes, for self-pay patients the Wegovy pill costs less than the Wegovy injection. NovoCare lists the pill at $149 to $299 a month and the pen at $349 a month after two $199 introductory fills. In separate trials the two produced similar average weight loss.

Key takeaways

  • Self-pay through NovoCare right now: the pill is $149, $199 or $299 a month by tablet strength. The pen is $199 for two starter fills, then $349 a month; Wegovy HD is $399.
  • In separate trials, the 25 mg pill produced 13.6% average weight loss at 64 weeks and the 2.4 mg pen 14.9% at 68 weeks.
  • Stomach side effects were common with both. In the pill's main trial, 74.0% reported them against 42.2% on placebo.
  • The pill is a daily tablet with an empty-stomach morning routine set by its label. The pen is one injection a week, with or without food.
  • Only the injection is approved for teens 12 and older, for the liver condition MASH and at the 7.2 mg dose.

FDA approved the Wegovy pill on December 22, 2025, four and a half years after the Wegovy pen [2]. Both contain semaglutide, and both share one prescribing label [1]. The choice comes down to three things: what you'll pay, whether a daily routine or a weekly shot fits your life, and whether you need one of the uses only the pen covers.

On self-pay, the pill costs less at every tablet strength

Novo Nordisk sells both forms direct to cash-paying patients through NovoCare Pharmacy. The pill's price rises with the tablet strength, and the top price is still $50 below the pen's regular price.

At the top tablet strength, a year of the pill comes to $3,588 by our arithmetic. A year on the pen at the regular price is $4,188, or $3,888 in a first year that starts with the two $199 fills [3]. To run these numbers with your own plan, use the monthly cost calculator. The full price picture, including insurance, is in what Wegovy costs.

Many people get either form through a telehealth program, which adds a clinical fee on top of the medication. The program's monthly figure below includes that fee.

The pill did nearly as well as the shot in its trial

The pill's main weight trial, OASIS 4, gave 307 adults with obesity or overweight the 25 mg tablet or placebo for 64 weeks. Average weight loss was 13.6% on the pill and 2.2% on placebo [4]. The pen's main trial, STEP 1, gave 1,961 adults the 2.4 mg weekly dose or placebo for 68 weeks: 14.9% against 2.4% [5].

Average weight loss in each form's main trial (separate trials)
Wegovy pill 25 mg, 64 weeks (OASIS 4)
13.6%[4]
Wegovy pen 2.4 mg, 68 weeks (STEP 1)
14.9%[5]

Those are separate trials of different sizes and lengths, so a 1.3-point gap doesn't mean the shot is stronger. Novo Nordisk funded both. What the numbers do say is that the pill isn't a weaker cousin: at its top strength it lands in the same range. Side effects were similar in kind. Among OASIS 4 participants on the pill, 74.0% reported stomach or bowel effects, against 42.2% on placebo [4]. The label reports severe stomach reactions in 2% of people on the tablets and 4.1% on the injection, from separate trials [1]. The 7.2 mg Wegovy HD pen is a separate, higher option that the pill doesn't match. More on the pen's evidence in the Wegovy guide.

Daily pill vs weekly shot: the routine is the real trade-off

The pen is one injection a week, on the same day, and the label says it can be taken with or without food [1]. The pill is one tablet every day, and the label sets conditions on how it's taken: in the morning, on an empty stomach, with a small amount of plain water, then a wait before food, drinks or other medicines [1].

The conditions exist because so little of the tablet gets absorbed. The label puts it at about 1% to 2%, mostly in the stomach, against 89% for the injection [1]. In practice, the pill suits people with a steady morning and no other pills that must come first. Your prescriber walks you through the exact routine. If you take thyroid medicine, say so: the label reports that a semaglutide tablet raised levothyroxine levels by a third in a study [1].

Both labels start low and build up. The pill starts at 1.5 mg a day and the usual maintenance tablet is 25 mg; the pen starts at 0.25 mg a week and the label allows up to 7.2 mg [1]. Your prescriber sets the pace.

From the combined Wegovy injection and tablets label on DailyMed (version 19). The label says the safety and effectiveness of the tablets have not been established in children or in people with MASH.

So a teenager, or anyone being treated for MASH, needs the injection. For adults whose goal is weight or heart risk, either form fits the label [1].

Storage and travel: the pill is simpler

Tablets stay at room temperature, 68°F to 77°F, in their original bottle to keep moisture out [1]. That makes the pill the easier one to travel with. Pens belong in the refrigerator, though the label allows up to 28 days between 46°F and 86°F before the cap comes off. For the details, see does Wegovy need to be refrigerated.

Insurance can change the math

NovoCare's cash prices are for people paying themselves. With commercial insurance that covers Wegovy, Novo Nordisk's savings card can cut what you pay to $25 a month, though it covers no more than $100 of savings a month [3]. Coverage of the pill and the pen can differ by plan, so ask your insurer about each by name.

If you're on Medicare Part D, CMS runs a temporary program from July 1, 2026, through December 31, 2027, called the Medicare GLP-1 Bridge. Eligible beneficiaries pay a $50 copay for certain GLP-1 drugs, and CMS sets who qualifies [6].

Switching between the pill and the shot

The Wegovy label includes directions for moving between the tablets and the injection in either direction [1]. They aren't a simple one-for-one swap, so let your prescriber handle the change and the starting strength. If cost is the reason, it's worth raising: on cash prices, a move from the pen to the top tablet strength saves $50 a month.

Two other pills often come up in the same conversation. Rybelsus and the newer Ozempic tablets are semaglutide pills for type 2 diabetes, not weight; they're covered in the oral semaglutide guide. Foundayo is Lilly's daily weight pill, a different molecule called orforglipron. If you're weighing Wegovy against Lilly's injection instead, see Zepbound vs Wegovy, and for the diabetes brand, Ozempic vs Wegovy.

Frequently asked

Is the Wegovy pill as effective as the shot?
Close. In separate trials the 25 mg pill produced 13.6% average weight loss at 64 weeks and the 2.4 mg pen 14.9% at 68 weeks. The pen also comes in a 7.2 mg dose the pill doesn't match.
How much is the Wegovy pill without insurance?
Through NovoCare, as of this update: each 30-tablet bottle is $149 at the 1.5 mg strength, $199 at 4 mg, and $299 at either 9 mg or 25 mg. NovoCare says prices will be updated after December 31, 2026.
Can teens take the Wegovy pill?
No. The tablets are approved for adults only. The Wegovy injection is the form approved for weight management in children 12 and older with obesity.
Is the Wegovy pill the same as Rybelsus?
No. Both contain semaglutide, but Rybelsus and Ozempic tablets are approved for type 2 diabetes, while the Wegovy pill is approved for weight and heart-risk reduction. They come in different strengths and aren't interchangeable.
Does the $199 intro price for the pen last?
No. It covers two monthly fills of the 0.25 mg or 0.5 mg pen for new patients, through December 31, 2026. After that the pen is $349 a month, and Wegovy HD is $399.
